Dr. Forrest was quite kind to perform Permanent Impairment Ratings for USDOL and my Schedule Award. Working in conjunction with Reports from Dr. Frank Graf the reports spanned 2 changes in AMA Guides, from editions 5, then 6, then the July/August Newsletter 2009. It was here that three Doctors bowed out and did not use the requested format.
