Know your numbers. Extend your life.
A biometric screening can help you learn more about your health. You can talk to your doctor about your screening results and ways to improve your health over time. During a screening, several measurements will be collected – such as your height, weight and blood pressure – and a sample of your blood will be used to determine your cholesterol, glucose levels and triglycerides. Your individual screening results will not be shared with your health plan.

- Your results will tell you more about your health risks and your overall well-being.
- You can speak to your doctor about ways to lower your risks over time.

Key dates
The 2025 screening period is from Jan. 1 – Nov. 30, 2025, however, if you complete a screening with your doctor anytime between Dec. 1, 2024 – Nov. 30, 2025, these lab results can count toward your 2025 incentive earnings. You have until Dec. 15, 2025, to submit a Physician Results Form if utilizing this option.
Prepare for your screening
Here is how you can make sure to have a successful screening experience:
- Although not required, we recommend fasting for at least nine hours prior to your screening; this includes all food and drinks except for water.
- Failure to properly fast may result in inaccurate blood test results. (If you have a health condition, such as diabetes or pregnancy, or any other medical concerns, consult a doctor before fasting.)
- Drink plenty of water to make it easier to give a sample of your blood.
- Continue taking medications as prescribed the day of the screening.*
- Arrive five minutes before your scheduled time.
- Allow about 20 minutes for your screening.
*Follow your doctor’s instructions for taking your medications, including taking them with food if necessary.
Incentives
Complete your screening by Nov. 30, 2025, to earn $50. If three out of four biometric screening lab values are in range, you will earn $125. If your blood pressure targets are also in range, you will earn an additional $25. Learn more about biometric screening target ranges here.
Incentives are available for active state and higher education employees and spouses who are enrolled in medical insurance with Partners for Health.

Partners for Health-sponsored screening events are held at different worksite locations throughout the year. Get screened by your doctor.
A Physician Results Form is a form that your doctor can complete during your next visit or exam. Download or print the form by clicking below. Mail your completed form to the return address or fax the form to the number listed. You can also upload your form online using the link below. If you complete a biometric screening with your doctor anytime between Dec. 1, 2024 – Nov. 30, 2025, these lab results can count toward your 2025 incentive earnings. You have until Dec. 15, 2025 to submit your Physician Results Form.

Get screened at a Quest Diagnostics PSC.
Visit a Quest Diagnostics Patient Service Center near you on or before Nov. 30, 2025, to complete your screening. You can schedule a screening by logging in or registering for the Partners for Health wellness program. Get screened at home.
Complete your screening at-home! Visit stateoftn.sharecare.com and select Achieve, Programs and Health Screening to access the Quest site. Then, select At-Home Test and Order Materials. When your kit arrives, follow its instructions to complete your screening. Then complete and return the kit by Nov. 30, 2025 to earn credit. Please note: The final day to order an at-home test kit for the 2025 program year is Nov. 2, 2025. These kits must be returned by Nov. 30, 2025. If available, a new kit will need to be ordered for subsequent years.
*If you order an at-home test kit and do not return it within the same year, you will no longer see it as an option to complete your biometric screening in subsequent program years.
