General

Who is eligible to participate in the Partners for Health wellness program?

Employees, spouses, adult dependents and retirees who are enrolled in medical insurance with Partners for Health have access to Sharecare’s tools and programs.
Incentives are available for active state and higher education employees and spouses only.

How do I set up my Sharecare account?

Visit stateoftn.sharecare.com to register and begin online activities. The first time you log in you will need to register. Steps to register can be found here – “How to Register” guide

Will I be charged for participating in this program?

No. This program is confidential, voluntary and offered at no additional cost to you as part of your health benefits.

Is my health information protected?

Yes. Your privacy is ensured in compliance with the Health Insurance Portability and Accountability Act of 1996. Sharecare adheres to strict security guidelines. None of your personal information will be shared with Partners for Health without your permission.

What information is shared with my employer or Partners for Health?

No individual participant information is shared with your employer. For purposes of paying the cash incentives, state payroll and your employer, if you work for University of Tennessee, a Tennessee Board of Regents school or a state offline agency, known as STOLA, your employer will have access to the dollar amount you earned in order to pay the incentive in your paycheck.

Can I opt out of participating in this program?

Yes. Participation is completely voluntary. If you would like to decline participation or opt out of Sharecare programs, please follow the steps below:

  • To disenroll from Lifestyle Management or Chronic Condition Management Coaching – Please call Customer Service at 888.741.3390 and indicate you would like to leave the program. You can also inform your coach or clinician during any interaction. If you opt out of these programs, you will still have access to Sharecare’s online programs, tools and incentives via stateoftn.sharecare.com.
  • To disenroll from Eat Right Now or Onduo – Please call Customer Service at 888.741.3390 and indicate you would like to leave the program. If you opt out of these programs, you will still have access to Sharecare’s online programs, tools and incentives via stateoftn.sharecare.com.
  • To opt out of all Sharecare programs, including the online tools and ability to earn incentives – Please call Customer Service at 888.741.3390. If you change your mind about participating, you must call and let Sharecare know.

How do I update my communication preferences?

  • From your computer:
    • Sign into your Sharecare account, then select the gear icon in the top right corner.
    • Visit the Communications page to update your choices for email subscription and/or text messages.
  • From your Android or Apple Smartphone:
    • Sign into your Sharecare account. Then select You from the bottom banner.
    • Select the gear icon and Communications.
    • Select the methods you wish to update (push notifications, email subscriptions and/or text messages).

I signed up for the Onduo Diabetes Management program. What do I need to do to receive the free at-home A1c test kit from Onduo?

Health care rules are different from state to state, so some Members enrolled in the Onduo Diabetes Management program will need to complete a Telemedicine visit with an Onduo provider before the free at-home A1c test kit will be mailed.

Telemedicine visits are free for Members enrolled in Onduo (up to 10 visits per year) and the visit is only required if you live in one of the following states:  Arizona, Arkansas, Delaware, District of Columbia, Indiana, Kansas, Louisiana, Minnesota, Mississippi, Missouri, New Mexico, Oklahoma, Rhode Island, Vermont, or West Virginia.

Members enrolled in Onduo who live in Tennessee (or another state not listed above), can have their at-home A1c test kit ordered without a Telemedicine visit.

RealAge® Test

What is the RealAge Test?

The RealAge Test is Sharecare’s scientifically-based health assessment that estimates how you are aging based on your lifestyle, health history, biometrics and more. It should only take 10-15 minutes to complete.

How do I review my RealAge Results?

You can access your RealAge Test results by selecting You and Health Profile. Once you have taken your RealAge Test, you’ll get tips on how to lower your health risks over time. You can also update or change your answers at any time.

Do I earn incentives for completing the RealAge Test?

If you are interested in earning wellness incentives, you must complete your RealAge Test by Dec. 31 to unlock your incentives for the current year. You will not receive any incentive payments until the RealAge Test is complete.

How do I complete the RealAge Test?

To complete the RealAge Test, visit stateoftn.sharecare.com. Once you create your account, select Get Started in the Welcome Message. If you have already created your account, log in and select You from the banner and Take the RealAge Test. For instructions on how to set up your online account, see question two under General.

If I don’t finish taking the RealAge Test, do I have to start over?

Your RealAge Test answers are automatically saved, so you can pause the test at any time. You can complete the RealAge Test at a later date, when ready.

Biometric Screenings

How much does a biometric screening cost?

There is no added cost for eligible members to complete their biometric screening in one of four ways:

  • Attend a Partners for Health sponsored on-site screening event
  • Get screened at a Quest Diagnostics Patient Service Center
  • Have your doctor complete a Physician Results Form
  • Complete an at-home test kit

Who administers the Partners for Health-sponsored onsite biometric screening?

Onsite screenings are administered by Sharecare and their screening vendor, Quest Diagnostics. Sharecare and Quest protect the confidentiality of your information in compliance with the requirements of the Health Insurance Portability and Accountability Act of 1996 and applicable state laws.

How do I prepare for the screening?

Here is how you can make sure to have a successful screening experience:

  • Although not required, we recommend fasting for at least nine hours prior to your screening. This includes all food and drinks except for water.
  • Failure to properly fast may cause inaccurate blood test results. If you have a health condition, such as diabetes or pregnancy, or any other medical concerns, consult a doctor before fasting.
  • Drink plenty of water to make it easier to give a sample of your blood.
  • Continue taking medications as prescribed the day of the screening.*
  • Arrive five minutes before your scheduled time.
  • Allow about 20 minutes for your screening.

*Follow your doctor’s instructions for taking your medications, including taking them with food if necessary.

Where can I view my biometric screening results?

To view your results, follow the below instructions:

  • From your computer:
  1. Visit stateoftn.sharecare.com and follow the prompts to login.
  2. Choose the You option from the menu choices on the left. This is the last icon with the person in the circle.
  3. Click on the Health Profile and select Biometrics to see your results.
  • From your Android or Apple smartphone using the Sharecare app:
  1. Open the app and follow the prompts to login.
  2. Choose the You icon from the bottom menu choices.
  3. Click on Health Profile and select Biometrics to see your results.

It can take between two to four weeks before you will see your results within your Sharecare Health Profile. If you use a 2025 Physician Results Form, we will not be able to display your results until your doctor’s office faxes the form to Sharecare.

What if I complete my biometric screening in December? Can I still earn the incentive?

The 2025 screening period will be from Jan. 1 – Nov. 30, 2025, however, if you complete a biometric screening with your doctor anytime between Dec. 1, 2024 – Nov. 30, 2025, these lab results can count toward your 2025 incentive earnings. You have until Dec. 15, 2025, to submit a Physician Results Form if utilizing this option.

Can I earn incentives for my lab values being in target range?

Yes! If you have three out of four values in the lower risk range for metabolic syndrome (see below) you will automatically earn a $125 incentive. You must first complete the RealAge Test and submit your biometric values to qualify. If three or more values are in the higher risk range for metabolic syndrome, you may be at higher risk for heart disease, diabetes or stroke. Some target ranges for healthiest or lowest risk may be different.

What if I don’t use my at-home test kit I ordered. Can I use it for the next program year?

Your at-home test kit must be ordered and returned in the same program year. For example, if you order the kit to use in 2025, you must return the kit by the Nov. 30, 2025 deadline. If you order an at-home test kit and do not return it within the same year, you will no longer see it as an option to complete your biometric screening in subsequent program years.

Preventive Screenings

What are the different preventive screenings that qualify for incentives?

Complete your annual physical or well woman exam to earn $25. Then, complete select preventive screenings to earn an additional $25 per screening:

  • Breast cancer screening for females ages 40 – 75
  • Colon cancer screenings for ages 45 – 75
  • Cervical cancer screenings for females ages 21 – 65
  • Lung cancer screening for adults ages 50 – 80 (with 20 pack-year history of smoking)

What if I complete my preventive screening in December? Can I still earn the incentive?

If you complete a preventive screening in December, it will count toward the upcoming year’s incentive earnings. For example:

  • Any preventive screenings completed in Dec. 2024, will count toward the 2025 program year incentive earnings.

What if I don’t qualify for a preventive screening because of my age or another reason? Can I receive a medical exemption?

Medical exemptions will not be offered for preventive screenings. Please review other wellness activities to earn the maximum incentive amount for 2025 (under Earn Incentives section).

Incentives

Who is eligible to receive the incentive?

Active state and higher education employees and spouses enrolled in medical insurance with Partners for Health can earn up to $250 per program year ($500 per household). The incentive will be deposited into the head of contract’s end of month paycheck by your employer. The incentive will be combined with your regular pay. The incentive is taxable and subject to withholding, garnishment and reporting and will impact the actual amount in your paycheck.

Sharecare is responsible for tracking and reporting your activities. All activities will be tracked through Sharecare’s website. You must first set up your account and complete the RealAge Test in order to start receiving credit for incentives.

When do my activities need to be complete to earn the incentive?

Activities need to be completed by Nov. 30 or Dec. 31, 2025 depending on the activity. Go to the Earn Incentives page for a complete list of eligible activities for 2025 rewards.

Where can I track my activities and incentives?

To see your 2025 incentive status, follow the instructions below:

  • From your computer:
    1. Go to stateoftn.sharecare.com and follow the prompts to login. If you have not registered and completed your RealAge Test, you will need to do so prior to viewing your activity status.
    2. Choose the Achieve icon on the left side of the menu.
    3. Choose the Rewards option and click on Partners for Health Wellness Program.
  • From your Android or Apple smartphone using the Sharecare app:
    1. Open the app
    2. Choose the Achieve icon from the bottom menu choices.
    3. Choose the Rewards option from this screen and click on Partners for Health Wellness Program.

How long after the activity shows complete in the Rewards Center will I see the money in my paycheck?

The cash payment for both the employee and the spouse will be paid in the head of contract’s paycheck. Once each of you complete the RealAge Test, payments will be made monthly and always in the end of month paycheck. For some activities it could take up to three months depending on the activity and when it is completed and reported to Sharecare. All payments should generally be made within a month or two of the activity being completed and reported to Sharecare. Payments for completing preventive exams usually take the longest to process.

Where can I find the payments?

You can track the payments on your digital paystub:
State employees: Your incentive will show in the Hours and Earnings section on the ESS paycheck with the description “Well Ince”
University of Tennessee employees: Your incentive will show in the gross pay section with the description “Wellness Incentives”
Tennessee Board of Regents school employees: Your incentive will show in earnings with the description “TBR Hlth”

What if I am a state employee and elected for my incentive payments to go to my HSA. How can I track the payments?

State employees enrolled in the Consumer-Driven Health Plan (CDHP) can also put their wellness program cash incentives into their HSA. State employees can make this choice during Annual Enrollment. Note: Any wellness incentives deposited into the HSA will count toward the overall HSA IRS annual maximum. This means you need to take the 2025 IRS limit and subtract the $500 or $1,000 seed funds from the state and subtract how much you think you will earn in wellness program incentives in 2025 to determine how much money you want to contribute to your account pretax.  State employees can use Edison employee self-service to adjust their HSA contribution at any time. To track your incentive payment, your incentive will show in the Hours and Earnings section on the ESS paycheck with the description “Well Ince” and you will also see the same amount in the Before-Tax Deductions section listed with the description “Health Savings Accnt Wellnss”. To view your HSA contributions on the Optum Bank website, you can view your contributions in the blue box and see your recent contributions at the bottom. Note, there is no filter specific to wellness contributions.

Sample ESS paycheck

If both the head of contract and spouse work for state/higher education, will each receive the incentive because we each receive a paycheck?

Even if your spouse is an employee, if he/she is a dependent on your health insurance, the head of contract will receive the incentive payment in his/her paycheck. If you have separate coverage, then you will each receive the cash payment in your own paycheck.

What happens if I become eligible during the year? Can I still earn the incentives?

Yes. However, new hires and new plan members may be limited in the dollar amount you can earn depending on your hire date and the activity deadlines.

What if I can’t participate due to a medical issue? Will I still be able to earn incentives?

Yes. The cash incentives are available to all eligible members. If you think you are unable to meet a standard for the cash incentive, you might qualify for an opportunity to earn the incentive through different means. Call Sharecare at 888.741.3390 and they will work with you.

If I am pregnant, can I earn my full incentive amount?

Yes. Once you complete the RealAge Test, you have the option to participate in various activities to help you earn the full incentive amount. Activities include participating in coaching or guided programs, wellness challenges, case management and more. See the full list of activities to earn your incentive under the “Earn Incentives” section.

Will I receive my incentive if I am out on an approved leave of absence such as short-term or long-term disability?

State Employees: The answer is yes. The type of leave you are on will determine how the payment is made. You will receive your incentive payment via a paper paycheck mailed to your current home address in Edison when on these types of approved leave: short-term or long-term disability, parental leave, military leave, seasonal closure and education leave.

You will receive your incentive pay on your regular paycheck when you are on these types of approved leave: paid leave of absence for division of claims, FMLA with pay, FMLA without pay, termination with pay and Sick Leave Bank.

Will I receive my incentives if I retire or terminate employment?

State employees: It depends on when the state receives notice of the incentive payment. Incentive payments must be processed in the current calendar year in which the incentive is earned. You will either receive the payment included with your other earnings on a regular paycheck or receive a paper paycheck in the mail, for the incentive payment amount. The method of payment is based on the length of time since your termination date and receipt of the incentive payment from the vendor.

Tennessee Board of Regents schools: Yes, you will receive payment via direct deposit either on a regular paycheck or as an incentive-only paycheck if no other pay is due.

What if I believe that I or my spouse completed activities, but I do not see the money in my paycheck?

First log into your Sharecare account to confirm you have completed the RealAge Test. Some activities could take up to three months to appear in your paycheck. If it has been longer than three months since the activity was completed, please call Sharecare at 888.741.3390.

How will the incentive impact my taxes?

The tax withholding will be subject to your tax bracket. The payment is also subject to garnishment.
